 The 2016 Community Health Needs Assessment provides an overview of findings from a community health needs assessment (CHNA) conducted on behalf of Logansport Memorial Hospital (LMH) in order to assess health needs in Cass County, Indiana. The assessment was initiated to identify the community’s most important health issues in order to develop an effective implementation strategy to address such needs.
untitled Each county has a Local Coordinating Council (LCC), which is the planning and coordinating body for addressing alcohol and other drug problems. Members include volunteers from a variety of organizations including education, treatment, social services, and local police.\

LCCs are required to submit a Comprehensive Community Plan (CCP) to the Commission, which consists of

  • An assessment of the local alcohol and other drug abuse problems in the county, including problem identification and supportive data;
  • A listing of proposed objectives to help alleviate the stated problems; and
  • An evaluation component designed to measure the success of the plan’s strategies.

yGL8yg89_400x400  The Indiana Young Adults Survey, developed by FSPH researchers and administered by ECPIP, was designed to collect information on substance use attitudes and behaviors of young adults aged 18-25, residing in the 10 PFS communities. Participant sampling was done through two waves of randomized text messaging with a link that allowed recipients to click into the web survey.

connecting_dots200w258h  This brief shares research on connections between different forms of violence and describes how these connections affect communities. The purpose is to support those working to prevent violence in thinking strategically and creatively about preventing all types of violence from occurring in the first place as well as coordinating and integrating responses to violence in a way that recognizes these connections and considers the individual in the context of their home environment, neighborhood, and larger community.
